Can I order home oxygen for my patient?

Hertfordshire: DO NOT PRESCRIBE – All patients should be referred to the community respiratory specialist team.

Community respiratory service in east and north Hertfordshire (provided by Hertfordshire Community NHS Trust together with East and North Hertfordshire NHS Trust) 

West Essex: ONLY PRESCRIBE for non-respiratory patients e.g. palliative care and cluster headache. All other patients should be referred to the community respiratory specialist team.

How do I order home oxygen?

You would need to complete HOOF Part A via the Home Oxygen Portal. Registration is quick and simple. Log on to BOC Home Oxygen Portal click "Register” and follow the instructions! Registrations are processed same day and you will receive email confirmation that you can now access the Portal.

I’m not sure how to complete the HOOF?

The Home Oxygen Portal should guide you through the process however if you have any questions how to order home oxygen for your patient please call the BOC clinician line on 0800 151 0138. Alternatively, you can visit the clinician’s section of the website for useful downloads, including the Initial Home Oxygen Risk Mitigation form (IHORM). If ordering for a new patient a HOOF cannot be submitted without completing an IHORM.

How often does the Initial Home Oxygen Risk Mitigation form (IHORM) need to be completed?

The IHORM should be completed:

· At initiation (a HOOF cannot be submitted without the IHORM)

· If any change in circumstances e.g. change of address

· Annually at review 

My patient no longer meets the criteria for home oxygen or doesn’t want it. How do I get it removed?

You can submit a removal request via the Home Oxygen Portal.
