·     Can I order home oxygen for my patient?

Hertfordshire: SPECIALIST PRESCRIBING ONLY – All adult patients (respiratory and non-respiratory conditions) should be referred to the community respiratory specialist team who will review patients within 2 weeks. General Practice can order oxygen in exceptional circumstances i.e. same day oxygen for end-of-life care.

Community respiratory service in East and North Hertfordshire (provided by Hertfordshire Community NHS Trust together with East and North Hertfordshire NHS Trust) 

West Essex: SPECIALIST PRESCRIBING for adult patients with a respiratory condition. Patients should be referred to the community respiratory specialist team who will review patients within 2 weeks. General Practice can order oxygen for patients with a non-respiratory condition e.g. palliative care and cluster headache. Emergency oxygen can be ordered in exceptional circumstances i.e. same day oxygen for end-of-life care.

·     How do I order home oxygen?

•  Register and log in to the Baywater Healthcare Online Portal click here.                                                                                                 
•  Complete HOOF Part A via the Baywater Healthcare Online Portal.

The Baywater Healthcare Online Portal should guide you through the process however if you have any questions how to order home oxygen for your patient, please call the Baywater Clinicians Helpline on 01270 218 050 available 8.00am – 6.30pm, seven days a week

•  If ordering for a new patient a HOOF cannot be submitted without completing a Home Oxygen Risk Mitigation form (IHORM).

·     Can I order ambulatory oxygen?

Same day ambulatory oxygen can be ordered on HOOF Part A for emergency end-of-life care ONLY. Clinical code 8 MUST be selected under Clinical Details (section 3.1) AND information needs to be added into the Additional Patient Information box (section 11). Please note the HOOF will be rejected if only ambulatory oxygen is ordered. See example below:

 

·     How often does the Initial Home Oxygen Risk Mitigation form (IHORM) need to be completed?             

The IHORM should be completed:

• At initiation (a HOOF cannot be submitted without the IHORM)
• If any change in circumstances e.g. change of address
• Annually at review

·     My patient no longer meets the criteria for home oxygen or doesn’t want it. How do I get it removed?

You can submit a removal request via the Baywater Healthcare Online Portal.
