Hypertension in Adults - Diagnosis and Initial Management

Next review date: September 2025. 

This is an integrated pathway for the diagnosis and initial management of hypertension in adults in GP and community pharmacy settings. The pathway aims to:

1. Provide updated guidance on the diagnosis and management of hypertension
2. Increase awareness of referral links between GP and community pharmacy for hypertension
3. Increase access to services for patients
4. Optimise identification, monitoring and treatment of hypertension

 For patients where the management of hypertension is proving difficult - consider advice and guidance instead of face-to-face out-patient clinic referral.
