Delgocitinib for treating moderate to severe chronic hand eczema

Indication: Treatment of moderate to severe chronic hand eczema (CHE) in adults.

HWE APC recommendation:

In line with NICE TA1107

  • Delgocitinib can be used, within its marketing authorisation, as an option to treat moderate to severe chronic hand eczema in adults when topical corticosteroids have not worked or are not suitable. Delgocitinib can only be used if the company provides it according to the commercial arrangement.
  • Delgocitinib should be started and monitored by a healthcare professional with experience in diagnosing and treating chronic hand eczema in secondary care.
  • Consider how skin colour could affect the assessment of severity and make any adjustments needed.
Assessing disease severity, response definition for continuation and stopping criteria to be undertaken in line with TA1107 and the market authorisation.  The market authorisation provides the following information:
  • A thin layer of delgocitinib should be applied twice daily to the affected skin of the hands and wrists until the skin is clear or almost clear. It is recommended to apply the cream at regular intervals, approximately 12 hours apart. Section 5.1 outlines the Investigators Global Assessment for chronic hand eczema (IGA-CHE) for use in assessing disease severity. [Clear, almost clear, mild, moderate, severe]
  • Treatment should be discontinued if no improvement is seen after 12 weeks of continuous treatment.
  • In the event of recurrence of the signs and symptoms of CHE (flares), twice daily treatment of the affected areas should be re-initiated as needed.
  • Shelf life of delgocitinib cream is 3 years and after first opening is 1 year.
