Acute care can be provided in the community or as same day emergency care (SDEC) to prevent deconditioning, reduce risk of delirium and falls, and hospital acquired infections.  

Acute care in the community includes Urgent Community Response/Rapid Response (providing care in peoples home within 2 hours), and Virtual Wards/ Hospital at Home. 

Please consider the risks of hospital admission for individuals, particularly those who are frail (Rockwood 4+), resident in a care home, or end-of-life. Check for advanced care plans.
