Acute Urgent/Emergency Frailty Pathway
If at all possible avoid hospital admission as ten days in a hosptial bed leads to ten years worth of loss of muscle mass in patients over 80 years old
- Follow any care plans in place. Respect advanced decisions
- Consider frailty hotline advice
- Consider referring to frailty clinic (to be seen within 24-48 hrs)
- Consider referring to community services / integrated hub / care coordination center, and use of virtual hospital.
- If not possible to manage in the community ensure that receiving hospital is given adequate information about patient's frailty status and care plan/ advanced decisions
